Only include projects within the venture that will help the health department improve and sustain data modernization over time. should be included: workforce capacity, leadership development, health department leadership, information system development and infrastructure development.
Once projects have been selected for implementation within a health department, the data modernization team should draft an overall summary of the data modernization venture as well as summaries of each project within the portfolio to communicate with partners about 1) what projects will be undertaken, 2) why that project is needed and 3) how the project will add value or improve a health department’s data capacity. Complete the Data Modernization Plan Project Summary worksheet with the DMI team as projects are being developed.
