Introduction
Welcome to the Clinical and Community Data Initiative (CODI) Toolbox!
This toolbox is for communities looking to enhance decision-making in health and design more effective interventions by connecting data across sectors. The CODI Toolbox provides a suite of tools designed for this purpose, addressing critical aspects such as partnership building, capacity assessments, and data linking and cleaning.
The toolbox is a comprehensive resource tailored for researchers, network partners, legal staff, implementers, and participants. Specifically designed for initiating, planning, implementing, and evaluating CODI pilot projects, the toolbox offers processes, standards, and tools to support these activities.
CODI, led by the Centers for Disease Control and Prevention (CDC), aims to build a community-driven information network that can link an individual’s data across clinical and community sectors into a single record. This infrastructure helps network members and others answer questions related to health, community programs, local policies to enhance childhood obesity research, surveillance, and evaluation.1
CODI brings together local people (individuals, organizations, and collaborations), processes (data assessments, sharing, and governance), and technology (record linkage, data models, and analytic programs). CODI establishes a locally owned infrastructure supporting community efforts to adopt data-driven approaches to improve health outcomes and achieve health equity.
Audience/Roles
The CODI Toolbox is designed for:
RESEARCHERS
Researchers and public health organizations that want to use individual tools, processes and standards to leverage the CODI architecture to request data
Network partners
Current and future CODI implementers including network partners
Legal staff
Legal staff that may want to view and use templates for CODI agreement
Implementers
Clinical care providers or community organizations seeking a deeper understanding of the health and data landscape in their region
Participants
Other partners who are interested in CODI or similar infrastructure
Approach
The contents of the CODI Toolbox are grouped into three categories: processes, standards, and tools.

Funding and Partners
Support for the development of the CODI Toolbox was provided in part by the Robert Wood Johnson Foundation (RWJF). The views expressed here do not necessarily reflect the views of RWJF. Some of this work was also supported in part by Cooperative Agreement number 6-NU38OT000316, funded by the Centers for Disease Control and Prevention (CDC). Its contents are solely the responsibility of the authors and do not necessarily represent the official views of the Centers for Disease Control and Prevention or the Department of Health and Human Services. This work was supported in part by the Office of the Secretary Patient-Centered Outcomes Research Trust Fund through an interagency agreement between the Office of the Assistant Secretary for Planning and Evaluation (ASPE) and the Centers for Disease Control and Prevention’s National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) (IAA #750120PE090053 and IAA #750120PE090053). The CODI Toolbox tools and processes were developed with ASPE and NCCDPHP funds.
We would like to acknowledge The MITRE Corporation for their contributions to the CODI project and toolbox. Visit the MITRE website for more information.
