The purpose of this document is to provide background information to public health agencies seeking to implement chronic disease surveillance using electronic health record (EHR) data, including considerations that should be discussed with clinical health information technology (IT) stakeholders, and suggest approaches for evaluating the reliability and validity of EHR-based surveillance indicators. While this document should aid planning for any chronic disease surveillance program, it utilizes specifics related to hypertension to illustrate important considerations.
