The Public Health Informatics Institute (PHII), in collaboration with the Centers for Disease Control and Prevention (CDC), established the Transitions Learning Community (TLC) to support programs that are planning to transition/transitioning to a new IIS platform or implementing other complex IT projects, such as new IIS modules, features or functionality. The TLC is an IIS community-driven initiative where jurisdictions meet, learn, discuss and benefit from each other.
The TLC leverages evidence-based tools and best practices from project management and change management disciplines to help jurisdictions understand their readiness for these changes and plan and manage the change process.
Monthly TLC webinars
The current TLC has active participants representing several jurisdictional IIS around the country. The TLC meets monthly in a virtual setting. Typically, calls take place on the fourth Wednesday of each month.
