Clinical and Community Data Initiative toolbox logo

Overview

The Clinical and Community Data Initiative (CODI) Toolbox is a compilation of processes, standards, and tools developed for researchers, network partners, legal staff, implementers, and participants to do their work. The toolbox is also designed for initiating, planning, implementing, and evaluating CODI pilot projects.

CODI aims to build a community-driven information network that can link an individual’s data across clinical and community sectors into a single record. CODI brings together data stored across different sectors and organizations to create individual-level, linked longitudinal records that include social determinants of health (SDOH), clinical and community interventions, and health outcomes. This infrastructure helps network members and others answer questions related to health, community programs, local policies to enhance childhood obesity research, surveillance, and evaluation.
