The purpose of this document is to provide guidance to public health agencies seeking to implement chronic disease surveillance using electronic health record (EHR) data, including considerations that should be discussed with clinical health information technology (IT) stakeholders, and suggest approaches for evaluating the reliability and validity of EHR-based surveillance indicators.
